We have two versions of our downloadable order form for you to use.

The first one is a .jpg designed to be printed straight from your browser and downloaded quickly. If you choose to use this .jpg, you will save some time, but please note that your printout may be somewhat blurry due to resolution differences in printing .jpgs from the web. The printout will be usable, but maybe not the sharpest quality possible.

The second one is a .pdf designed to be viewed and printed with the Adobe Acrobat Reader. This file will take longer to download, but will give you the print quality that we ideally want you to have. If you intend on making copies of our forms to distribute to people for us, we strongly advise using this file and not the smaller one.
